Legalization of Medical Reports

Legalization at the Consular Section

Complete a legalization application (select the link to get the form) and enclose the following:

·         Medical report legalized by a notary public, then signed by the county clerk wherever required (New York State), and later authenticated by the Secretary of the State where the affidavit is drafted, and a copy of the said documents.

·         Valid passport, Lebanese identification card (I.D.), or family/individual ikhraj kayd issued within the last 10 years and a copy of the said document.

·         Fees: $15.00 for each school certificate/school transcript (only cash or money orders payable to the Consulate General of Lebanon are accepted).

·         Certified self-addressed and stamped envelope for return (no return fees are accepted) if the documents are to be returned by mail.
