At the Consular Section, complete a travel document renewal application (select the link to get the form)
· 2 recent passport photos 1.9x1.4 inch with full face and white background.
· Lebanese identification card I.D. for Palestinians (tazkarat al hawiya) and two copies of the said document.
· Current travel document and two copies of its first 6 pages.
· Where applicable, copy of current foreign passport.
· UNRWA card (bitakat Iashi) and two copies of the said document.
· Copy of a US driver's license, utility bill or bank statement for proof of address.
· Parental consent form (select the link to get the form)
· Fingerprint of the right-hand thumb on a specified Consulate form.
· Fees: $40 for 1 year and $120.00 for 3 years (only cash or money orders payable to the Consulate General of Lebanon are accepted).
· Certified self-addressed and stamped envelope for return (no return fees are accepted) if the passport is to be returned by mail
