Apply for a travel document only to travel to Lebanon and only for one trip for a period of 3 months.
At the Consular Section, complete a travel document application (select the link to get the form) and enclose the following:
· 4 recent passport photos 1.9x1.4 inch with full face and white background.
· Copy of your Lebanese id or ikhraj kayd issued within the last 10 years.
· Current passport and a copy of its first 6 pages.
· Parental consent form (select the link to get the form) for minors and applicants under 18 signed by both parents or legal guardians, at the Consular Section or authenticated by a notary public, affirming their consent to the application.
· Fingerprint of the right-hand thumb on a specified Consulate form.
· Copy of a US driver's license, utility bill or bank statement for proof of address.
· Fees: $40.00 (only cash or money orders payable to the Consulate General of Lebanon are accepted).
· Certified self-addressed and stamped envelope for return (no return fees are accepted) if the travel document is to be delivered by mail.
If all requirements are met, the travel document will be issued on the same day.
Via certified mail, if you can’t get to the Consulate send us via certified mail:

· Notarized fingerprint of the right-hand thumb.
· Copy of a US driver's license, utility bill or bank statement for proof of address.

· Certified self-addressed and stamped envelope for return (no return fees are accepted).
If all requirements are met, we will deliver the issued travel document on the same day.
