At the Consular Section
Complete a visa application (select the link to get the form)
· Valid passport for a minimum of 6 months and a copy of its first 3 pages.
· Copy of Green Card or valid US visa for non US citizens.
· One recent passport photo 1.9x1.4 inch with full face and white background.
· Copy of a US driver's license, utility bill or bank statement for proof of address.
· Letter of employment stating the purpose of the trip.
· Letter from the business contact in Lebanon stating the purpose of the trip.
· Fees: $35.00 for single entry visas and $70.00 for multiple entry visas (only cash or money orders payable to the Consulate General of Lebanon are accepted).
· Certified self-addressed and stamped envelope for return (no return fees are accepted) if the passport is to be returned by mail.
